Whether it’s your first appointment or a routine check-in, we want you to feel prepared and supported. Patients are encouraged to fill out their forms ahead of time when possible.
This helps us check you in quickly and allows your appointment to begin on time. If you have questions while completing your paperwork, our team is happy to help.
We see patients beginning at age five. For your first visit, please bring:
Your vision and medical insurance cards
Any glasses or contact lenses you currently use (including sunglasses or specialty eyewear)
A list of current medications or relevant health history
If you need help accessing forms or aren’t sure what to bring, just call; we’re here to make your experience as smooth as possible.

We serve patients at three locations: Englewood and Osprey (inside Walmart), and Sarasota (inside SEE Optical). All locations are wheelchair-accessible and offer care by appointment.
